Join our dedicated Leaving Care Team in Harrow as an Advanced Practitioner, where you will play a pivotal role in supporting young people transitioning from care to independent living. In this fulfilling position, you will work closely with vulnerable young adults, offering guidance, emotional support, and practical assistance to ensure a smooth transition into adulthood. Your expertise will help empower these individuals to develop the skills and confidence necessary for successful integration into society.

As an Advanced Practitioner, your key responsibilities will include:

Conducting assessments and developing tailored care plans for young people leaving care. Providing direct support and advocacy, ensuring that the voices of young people are heard. Collaborating with multi-agency teams to facilitate access to housing, education, and employment opportunities. Supervising and mentoring junior social workers and students, fostering a culture of continuous learning. Maintaining comprehensive case records and ensuring compliance with relevant legislation and policies.

The ideal candidate will possess essential qualifications and skills, including:

A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Current registration with Social Work England. Significant experience working with young people in a social work setting, particularly in leaving care services. Strong assessment, analytical, and communication skills. The ability to work independently and as part of a team, demonstrating excellent leadership capabilities.
