As an Advanced Practitioner, your key responsibilities will include:
Conducting assessments and developing tailored care plans for young people leaving care. Providing direct support and advocacy, ensuring that the voices of young people are heard. Collaborating with multi-agency teams to facilitate access to housing, education, and employment opportunities. Supervising and mentoring junior social workers and students, fostering a culture of continuous learning. Maintaining comprehensive case records and ensuring compliance with relevant legislation and policies.The ideal candidate will possess essential qualifications and skills, including:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Current registration with Social Work England. Significant experience working with young people in a social work setting, particularly in leaving care services. Strong assessment, analytical, and communication skills.
