We are seeking a dedicated and experienced Children's AP for our Assessment Team in Bolton. As a key member of our team, you will be responsible for conducting thorough assessments of children and families to determine their needs and develop appropriate support plans.
Key responsibilities and duties include:
Conducting comprehensive assessments of children and families to identify risks and protective factors Developing and implementing care plans in collaboration with other professionals and agencies Providing direct support and intervention to children and families to promote their well-being and safety Participating in case conferences and reviews to ensure the ongoing effectiveness of support plans Maintaining accurate and up-to-date case records and reportsThe essential qualifications and skills for this role include:
Qualified Social Worker with relevant experience in children's services Registration with the Health and Care Professions Council (HCPC) Demonstrated knowledge of relevant legislation and policies related to child protection Strong assessment and analytical skills Excellent communication and interpersonal abilitiesAdditional requirements for this position may include a valid driver's license and access to a vehicle for travel within the community.
