Join our dedicated team as a Children's Assessment and Planning (AP) Social Worker within the Central District Team in Oldham. We are looking for passionate individuals who are committed to improving the lives of children and families in our community. As a key member of our team, you will conduct assessments, develop care plans, and work collaboratively with families and other professionals to ensure the best outcomes for children.
Your key responsibilities will include:
Conducting comprehensive assessments of children and families to identify needs and risks. Developing and implementing effective care plans in partnership with families and relevant stakeholders. Maintaining accurate and up-to-date case records in compliance with local and national policies. Participating in multi-agency meetings and contributing to safeguarding procedures. Providing support and guidance to families, ensuring their voices are heard in the decision-making process.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience in children's social work, particularly in assessment and planning. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
