We are seeking a dedicated and compassionate Children's AP - PSW Team member to join our dynamic team in Bolton. Key responsibilities include:
Conducting assessments and developing personalized care plans for children and families. Providing direct support to children, helping them navigate challenges and achieve their goals. Working collaboratively with multidisciplinary teams, including educators, healthcare professionals, and community organizations. Maintaining accurate and timely documentation of case notes and progress reports. Participating in regular supervision and training sessions to enhance your professional skills.To qualify for this position, candidates must possess:
A recognized qualification in Social Work or equivalent. Experience working with children and families in a social work capacity.
