As a Children’s Case Manager, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of children and families. Developing and implementing individualized support plans in collaboration with families and other professionals. Monitoring and reviewing progress, providing ongoing support and intervention as needed. Facilitating multi-agency meetings to ensure a coordinated approach to service delivery. Maintaining accurate and up-to-date case records in compliance with relevant legislation and policies.To be successful in this role, you will need to meet the following essential qualifications and skills:
A recognized social work qualification (CQSW, DipSW, or equivalent). Registration with Social Work England. Proven experience working with children and families in a social work setting. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
