In this role, you will be responsible for:
Conducting assessments and developing care plans tailored to the individual needs of children with disabilities. Providing direct support and guidance to families, helping them navigate available services and resources. Collaborating with multidisciplinary teams, including educators, healthcare providers, and community organizations, to ensure holistic support. Monitoring and reviewing progress, adjusting care plans as necessary to meet changing needs. Maintaining accurate and up-to-date case records and documentation in compliance with local policies.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Registered with Social Work England. Experience working with children and families, particularly those with disabilities. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
