Your key responsibilities will include:
Conducting comprehensive assessments to identify the needs of children with disabilities. Developing and implementing tailored care plans in collaboration with families and multi-agency partners. Providing ongoing support and intervention to families, ensuring access to necessary services and resources. Maintaining accurate and up-to-date case records in compliance with local authority policies. Participating in team meetings, supervision, and training to enhance professional development and service delivery.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in children's social work, particularly with children with disabilities. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
