We are seeking a dedicated and compassionate Qualified Social Worker to join our Children's Discharge Planning Team in Staffordshire. This role offers an exciting opportunity to work collaboratively with families, children, and multi-disciplinary teams to ensure smooth transitions from care settings back to home or appropriate placements. You will be instrumental in safeguarding the welfare of children and supporting families through the discharge process.
Key responsibilities include:
Conducting comprehensive assessments to identify the needs of children and families. Developing and implementing discharge plans in collaboration with healthcare professionals and family members. Providing ongoing support and guidance to families during the transition process. Ensuring compliance with local policies and statutory requirements. Maintaining accurate and up-to-date case records and documentation.To be considered for this position, candidates must possess the following qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Proven experience in children's social work, particularly in discharge planning or related fields. Strong communication and interpersonal skills, with the ability to engage effectively with children and families.
