Join our dedicated Leaving Care Team in Lincolnshire as a Children's Social Worker, where you will play a vital role in supporting young individuals transitioning from care to independence. In this rewarding position, you will work closely with care leavers, ensuring they receive the necessary guidance, support, and resources to thrive in their new environments. Your expertise will help empower these young people to achieve their goals and navigate the challenges of adulthood.
Key responsibilities include:
Conducting assessments and developing tailored care plans for young individuals. Providing direct support and advocacy to care leavers, ensuring their voices are heard. Collaborating with multi-agency partners to coordinate services and resources. Monitoring and reviewing the progress of care leavers, making adjustments to plans as necessary. Delivering workshops and training sessions on life skills to equip young people for independence.To excel in this role, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work England registration. Experience working with children and young people, particularly in a leaving care context. Strong communication and interpersonal skills, with a compassionate approach to care.
