Join our dedicated Transitions Team in Dudley as a Children's Social Worker, where you will play a vital role in supporting children and families during critical transition periods. As a key member of our team, you will work collaboratively with families, schools, and other professionals to ensure that children receive the support they need to thrive. Your expertise will help facilitate smooth transitions for children moving between care settings, schools, or into adulthood.
Your key responsibilities will include:
Conducting assessments to identify the needs of children and families. Developing and implementing care plans in collaboration with children, families, and other stakeholders. Providing direct support and guidance to children and families during transitions. Maintaining accurate records and documentation in accordance with policy and legislation. Participating in multi-agency meetings and contributing to the development of integrated services.To be successful in this role, you must possess essential qualifications and skills, including:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience working with children and families, particularly in transition settings. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
