Join our dedicated Children's TM - C&F Assessment Team in Durham, where you will play a pivotal role in safeguarding and enhancing the lives of children and families. As a Qualified Social Worker, you will conduct thorough assessments, develop care plans, and work collaboratively with families and external agencies to ensure the best outcomes for children in our community. Your expertise will be crucial in identifying risks and implementing effective interventions to support vulnerable families.
Your key responsibilities will include:
Conducting comprehensive assessments of children and families in need of support. Developing and reviewing care plans in collaboration with families and multidisciplinary teams. Providing ongoing support and guidance to families, ensuring they have access to necessary resources. Maintaining accurate and up-to-date case records and documentation. Participating in team meetings and case discussions to promote best practices and continuous improvement.To be successful in this role, you will need the following qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work, CQSW, DipSW). Registration with Social Work England. Proven experience in children’s social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with families.
