An exciting opportunity is available for a qualified Social Worker to join a busy Hospital Discharge Team in Stoke-on-Trent. You will support adults leaving hospital, helping them return home or move to suitable ongoing care safely, promptly and in line with their wishes, strengths and assessed needs.
You will complete timely, person-centred assessments, develop care and support plans, and coordinate discharge arrangements with individuals, families, carers and professionals. Working closely with hospital staff, health colleagues, community services and care providers, you will identify appropriate packages of support, make referrals and ensure that risks are carefully considered and managed.
The role will involve undertaking statutory social work duties, including reviewing arrangements, safeguarding adults, promoting independence and applying relevant legislation and local procedures. You will be expected to maintain accurate, comprehensive records, provide clear professional recommendations and manage a caseload effectively in a fast-paced environment. Supporting people to achieve positive outcomes and avoid unnecessary delays in hospital will be central to your work.
Applicants must hold a recognised Social Work qualification and current registration with Social Work England. You should have experience of adult social care, hospital discharge, assessment and care planning, together with a sound understanding of safeguarding, mental capacity and relevant social care legislation. Newly qualified Social Workers with suitable placement experience may also be considered.
We are looking for a confident and compassionate professional who can communicate effectively with people from a range of backgrounds. You will need strong organisational skills, the ability to prioritise competing demands and confidence working independently as well as part of a multidisciplinary team. A commitment to strengths-based practice, equality, dignity and continuous improvement is essential.
This is an opportunity to make a meaningful difference at a crucial point in people’s lives, helping individuals regain independence and ensuring they receive the right support at the right time.
