Join our dedicated Hull - TM - Assessment Team as a Qualified Social Worker, where you will play a crucial role in supporting individuals and families in need. In this position, you will be responsible for conducting comprehensive assessments, developing tailored care plans, and ensuring the safety and well-being of vulnerable populations. Your expertise will be vital in collaborating with multidisciplinary teams and external agencies to deliver high-quality social care services.
Your key responsibilities will include:
Conducting thorough assessments of individuals and families to identify needs and risks. Developing and implementing effective care plans in collaboration with service users and their support networks. Providing ongoing support and monitoring to ensure the effectiveness of interventions. Maintaining accurate and up-to-date case records and documentation. Participating in team meetings and case conferences to discuss progress and share insights.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience in social work, particularly in assessment and intervention. Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.
