Key responsibilities and duties include:

Conducting assessments and developing care plans for children and families Providing direct support and intervention to improve outcomes for service users Working collaboratively with other professionals and agencies to ensure holistic support Participating in case conferences, reviews, and court hearings as required Keeping accurate and up-to-date records of all interventions and interactions

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with Social Work England Proven experience in working within a Through Care Team or similar setting Strong assessment and analytical skills with the ability to make informed decisions Excellent communication and interpersonal skills to engage effectively with service users and stakeholders Ability to work autonomously and as part of a team in a fast-paced environment
