Your key responsibilities will include:
Conducting comprehensive assessments of children and their families to identify needs and strengths. Developing and implementing care plans that promote the well-being and development of children with disabilities. Working collaboratively with schools, healthcare providers, and other agencies to coordinate support services. Maintaining accurate and timely documentation in accordance with local policies and procedures. Providing guidance and support to families to empower them in advocating for their children's needs.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Care Wales (or eligibility to register). Demonstrated experience in children's social work, particularly with children with disabilities. Strong communication and interpersonal skills, with the ability to build relationships with children and families.
