We are currently seeking a dedicated and experienced Qualified Social Worker to join our team at the Cheshire East - TM - Children with Disabilities Hub. As a vital member of our team, you will be responsible for providing high-quality support and services to children with disabilities and their families.

Key responsibilities and duties include:

Conducting assessments to determine the needs of children with disabilities and developing care plans accordingly. Working collaboratively with other professionals, such as healthcare providers and educators, to ensure comprehensive support for children and families. Providing ongoing support and guidance to families, empowering them to navigate services and resources effectively. Advocating for the rights and best interests of children with disabilities within the community and with relevant agencies. Participating in multidisciplinary team meetings and case conferences to coordinate care and interventions.

Essential qualifications and skills:

Qualified Social Worker with a relevant degree and registration with the appropriate regulatory body. Demonstrated experience working with children with disabilities and their families in a social work capacity. Strong assessment and case management skills, with the ability to develop and implement effective care plans. Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse populations. Knowledge of relevant legislation and policies related to children with disabilities and safeguarding.
