We are currently seeking a dedicated and compassionate Stoke - AP - Leaving Care Team member to join our Care team. As a key member of the team, you will be responsible for providing support and guidance to young people transitioning from care to independent living.

Key responsibilities and duties:

Supporting young people in developing independent living skills such as budgeting, cooking, and accessing education or employment opportunities. Creating and implementing personalized care plans to meet the individual needs and goals of each young person. Advocating for the rights and well-being of young people in care, ensuring they have a voice in decision-making processes. Collaborating with external agencies and professionals to provide comprehensive support and services to young people. Participating in team meetings, case conferences, and training sessions to enhance professional development and knowledge.

Essential qualifications and skills:

Qualified in Social Work, Youth Work, or a related field with relevant experience in working with young people in care. Demonstrated understanding of the challenges faced by young people leaving care and the ability to provide effective support and guidance. Strong communication and interpersonal skills to build positive relationships with young people, colleagues, and external stakeholders. Ability to work independently and as part of a team, demonstrating flexibility, resilience, and a proactive approach to problem-solving. Knowledge of relevant legislation and policies related to leaving care services and safeguarding procedures.

Additional requirements and benefits:

Valid driver's license and access to a vehicle for work-related travel. Enhanced DBS clearance and willingness to undergo additional background checks as required.
