Join our dedicated Older Adults Community Team in Totton, Hampshire, as a Social Worker and make a meaningful impact in the lives of older adults within our community. In this role, you will be responsible for assessing the needs of older individuals, developing tailored care plans, and providing ongoing support to enhance their quality of life. Your expertise will help empower clients and their families to navigate complex social services and access necessary resources.

Your key responsibilities will include:

Conducting comprehensive assessments of older adults’ needs and circumstances. Collaborating with healthcare professionals, families, and community organizations to ensure holistic support. Developing and implementing individualized care plans that promote independence and well-being. Advocating for clients and facilitating access to services such as housing, financial aid, and healthcare. Providing crisis intervention and ongoing case management to ensure continuity of care.

To be successful in this role, you must possess the following qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Experience working with older adults in a community or health care setting. Strong communication and interpersonal skills, with a compassionate approach to client care. Ability to work independently and as part of a multidisciplinary team.
