Are you an experienced practitioner looking to make a meaningful impact in your community? We are seeking a dedicated Advance Practitioner to join our Locality Team in Leeds. In this pivotal role, you will work collaboratively with a multidisciplinary team to deliver high-quality care and support to individuals and families, ensuring their needs are met effectively and compassionately.

Your key responsibilities will include:

Leading assessments and care planning for complex cases, ensuring adherence to best practices and legislation. Providing supervision and mentorship to junior staff and students, fostering a culture of continuous learning and development. Collaborating with external agencies and stakeholders to promote integrated service delivery. Conducting reviews and evaluations of care plans to ensure optimal outcomes for service users. Participating in local initiatives and projects to enhance the quality of care within the community.

To be considered for this role, you must possess the following qualifications and skills:

A relevant professional qualification in Social Work, Nursing, or a related field. Registered with the appropriate regulatory body (e.g., Social Work England, NMC). Significant experience in a similar role within a care setting. Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations. Strong analytical and problem-solving abilities, with a commitment to evidence-based practice.
