Join our dynamic Assessment Team as a Social Worker in Barking & Dagenham, where your expertise will make a meaningful difference in the lives of individuals and families. In this role, you will play a crucial part in assessing the needs of vulnerable populations, ensuring they receive the appropriate support and resources. Your work will involve conducting thorough assessments, developing personalized care plans, and collaborating with various stakeholders to promote the well-being of those in your care.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals and families to identify their needs and strengths. Developing and implementing tailored support plans in collaboration with service users and their families. Working closely with multidisciplinary teams to ensure holistic support is provided. Maintaining accurate and up-to-date case records in compliance with local and national regulations. Providing guidance and support to less experienced team members and participating in team meetings and training.To be successful in this role, you will need to possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in social work, particularly in assessment and care planning. Strong understanding of relevant legislation and safeguarding policies.
