Your key responsibilities will include:
Conducting assessments to identify the needs of children with disabilities and their families. Developing and implementing care plans tailored to individual needs. Collaborating with multidisciplinary teams, including healthcare professionals, educators, and community organizations. Providing ongoing support and advocacy for families, helping them navigate available resources. Maintaining accurate and up-to-date case records and documentation.To be considered for this position, you must possess the following qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with children with disabilities and their families. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
