We are seeking a dedicated and compassionate Social Worker to join our Community Integration and Care (CIC) Team in Nottingham. This role is vital in providing support and assistance to individuals and families in need, helping them navigate complex social challenges and access essential services. As a key member of our team, you will work collaboratively with various stakeholders to promote the well-being and independence of our clients.
Your key responsibilities will include:
Conducting assessments to identify the needs of individuals and families. Developing and implementing care plans that promote social inclusion and enhance quality of life. Providing emotional support and guidance to clients and their families. Collaborating with other professionals, including healthcare providers and community organizations, to coordinate services. Monitoring and reviewing client progress, adjusting care plans as necessary. Maintaining accurate and up-to-date case records in compliance with legislation and policy.To be considered for this position, you must possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., BA/MA in Social Work). Registered with Social Work England. Demonstrated experience in social work practice, particularly within community settings. Strong communication and interpersonal skills, with the ability to build rapport with clients. Excellent organizational skills and the ability to manage a caseload effectively. A commitment to continuous professional development and adherence to ethical standards.Additional requirements for this role include a valid driving license and access to a vehicle, as travel within the community is essential.
