Join our dedicated CWD Team in West Sussex (Chichester) as a Social Worker, where you will play a vital role in supporting children with disabilities and their families. In this position, you will work collaboratively with multidisciplinary teams to ensure the best outcomes for children and young people. Your responsibilities will include conducting assessments, developing care plans, and providing ongoing support and intervention to families in need.
Key responsibilities and duties include:
Conducting comprehensive assessments of children and families to identify needs and strengths. Developing and implementing tailored care plans in collaboration with families and other professionals. Providing direct support and guidance to children and families, ensuring their voices are heard. Maintaining accurate and up-to-date case records, ensuring compliance with relevant legislation and policies. Participating in team meetings and contributing to the development of best practice within the team.The ideal candidate will possess the following essential qualifications and skills:
A recognised social work qualification (e.g., CQSW, DipSW, or equivalent). Registration with Social Work England. Experience working with children and families, particularly those with disabilities. Strong communication and interpersonal skills, with the ability to build rapport with diverse clients.
