Join our Leaving Care Team in Hackney, where you will play a crucial role in supporting care leavers as they navigate this critical phase of their lives. We are seeking a qualified social worker who is passionate about empowering young individuals, ensuring they have the resources and support needed for a successful transition into adulthood.

As a Social Worker in the Leaving Care Team, your key responsibilities will include:

Conducting assessments and developing tailored support plans for care leavers. Providing practical and emotional support to young people, including housing, education, and employment guidance. Building trusting relationships with young people, encouraging their independence and resilience. Collaborating with various stakeholders, including local authorities, educational institutions, and community organizations. Monitoring progress and adapting support plans as necessary to meet the evolving needs of young people.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Registered with Social Work England. Experience working with young people, particularly in a leaving care context. Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations. Excellent organizational skills and the ability to manage a caseload effectively.
