Join our dedicated Discharge Planning Team as a Qualified Social Worker (QSW) in Staffordshire, where you will play a vital role in supporting individuals during their transition from hospital to home. As a key member of our multidisciplinary team, you will be responsible for assessing the needs of patients, coordinating their discharge plans, and ensuring they receive the necessary support and services to facilitate a smooth recovery.
Your key responsibilities will include:
Conducting comprehensive assessments of patients' social, emotional, and practical needs. Collaborating with healthcare professionals to develop and implement effective discharge plans. Providing information and support to patients and their families regarding available resources and services. Monitoring and reviewing discharge plans to ensure ongoing support and adjustments as necessary. Maintaining accurate records and documentation in accordance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Proven experience in social work, particularly in hospital or discharge planning settings. Strong communication and interpersonal skills, with the ability to work collaboratively in a team.
