Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and risks. Developing, implementing, and reviewing care plans in collaboration with service users and other professionals. Providing direct support and guidance to individuals and families, ensuring access to resources and services. Maintaining accurate and timely records in accordance with legal and organizational standards. Participating in multi-agency meetings and contributing to the development of community initiatives.The ideal candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (BA/MA in Social Work or equivalent). Registered with Social Work England. Proven experience in a social work setting, preferably within a community or neighbourhood context. Strong assessment, communication, and interpersonal skills.
