Join our dedicated Leaving Care Team in Wrexham as a Qualified Social Worker (QSW) and make a meaningful impact in the lives of young individuals transitioning from care to independence. We are seeking a passionate and skilled professional who is committed to empowering young people, providing them with the support and guidance they need to thrive in their new environments.

As a QSW in our Leaving Care Team, your key responsibilities will include:

Conducting assessments to identify the needs and strengths of young people in care. Developing and implementing personalized care plans that promote independence and well-being. Providing ongoing support and advocacy to young people, ensuring they have access to essential services and resources. Collaborating with multidisciplinary teams, including education and health professionals, to ensure holistic support. Monitoring progress and adjusting care plans as necessary to meet evolving needs.

The ideal candidate will possess the following essential qualifications and skills:

A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work registration with Social Care Wales. Experience working with young people in a leaving care or similar setting. Strong communication and interpersonal skills, with the ability to build rapport with young individuals and families. Knowledge of relevant legislation and best practices in social work.
