Join our dedicated team as a Qualified Social Worker in the Children's Social Work - Children with Disabilities (CWD) Team in Southwark. We are seeking an experienced and compassionate professional who is committed to making a positive impact on the lives of children with disabilities and their families. In this role, you will work collaboratively with a multi-disciplinary team to assess needs, develop care plans, and ensure the delivery of high-quality services tailored to each child's unique circumstances.
Your key responsibilities will include:
Conducting comprehensive assessments of children and families to identify strengths, needs, and risks. Developing and implementing individualized support plans in partnership with families and other professionals. Providing direct support and guidance to children with disabilities and their families, ensuring their voices are heard. Monitoring and reviewing the effectiveness of support services, making necessary adjustments to care plans. Maintaining accurate records and documentation in compliance with statutory requirements and organizational policies.To be successful in this role, you will need:
A recognized social work qualification (BA/MA in Social Work or equivalent). Current Social Work England registration. Experience working within children's services, particularly with children with disabilities. Strong assessment and analytical skills, with the ability to make sound decisions under pressure.
