Social Worker - Single Point of Access Team- (Kingston & Richmond)

We are seeking a dedicated and compassionate Social Worker to join our Single Point of Access Team in Kingston & Richmond. This role is pivotal in providing high-quality support to individuals and families in need, ensuring they receive the appropriate services and interventions. The ideal candidate will be committed to promoting well-being and safeguarding vulnerable populations while working collaboratively with various stakeholders.

Key responsibilities include:

  • Conducting thorough assessments of clients' needs and circumstances.
  • Developing and implementing tailored support plans in collaboration with clients and their families.
  • Coordinating with healthcare providers, community organizations, and other professionals to facilitate access to services.
  • Providing crisis intervention and emotional support to clients and their families.
  • Maintaining accurate and up-to-date case records and documentation in compliance with organizational policies.

The successful candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (Degree in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience in a similar role, preferably within a multi-disciplinary team.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • Excellent assessment and analytical skills, with a solution-focused approach to problem-solving.
