Key responsibilities and duties:
- Conducting assessments and developing care plans for service users
- Providing support and intervention to promote positive outcomes
- Collaborating with multi-disciplinary teams and external agencies

- Participating in case conferences and reviews
Essential qualifications and skills:
- Qualified Social Worker with relevant professional registration
- Experience working within a Locality - AP setting
