We are currently seeking a dedicated and experienced Assessment Social Worker to join our team in Middlesbrough. As a key member of our Qualified Social Work field, you will be responsible for conducting assessments and providing support to individuals and families in need.
Key responsibilities and duties:
- Conducting comprehensive assessments to identify the needs of service users.
- Developing and implementing care plans to address identified needs and promote positive outcomes.
- Working collaboratively with multi-disciplinary teams and partner agencies to ensure holistic support for service users.
- Providing advocacy and support to empower service users in making informed decisions about their care.
- Participating in case conferences, reviews, and court proceedings as required.
Essential qualifications and skills:
- Qualified Social Worker with a relevant social work degree and HCPC registration.
