Your key responsibilities will include:

  • Assessing and identifying the needs of care leavers, creating tailored support plans.

  • Building strong, trusting relationships with care leavers to foster their personal development and resilience.
  • Collaborating with external agencies, stakeholders, and community organizations to ensure holistic support for young people.
  • Maintaining accurate records and documentation in accordance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience working with children and young people, particularly care leavers.
  • Strong communication and interpersonal skills, with the ability to engage effectively with young people from diverse backgrounds.
  • Excellent organizational skills and the ability to manage a caseload effectively.
