Children's TM - CWD Team - Nottingham
We are seeking a dedicated and passionate Children’s Team Member to join our Children with Disabilities (CWD) Team in Nottingham. This role is pivotal in supporting children and young people with disabilities and their families, ensuring they receive the best possible care and support. As a Qualified Social Worker, you will work collaboratively with families, schools, and other professionals to promote the welfare, safety, and development of children in our care.
Your key responsibilities will include:
- Conducting assessments to identify the needs of children with disabilities and their families.
- Developing and implementing tailored support plans that promote the well-being and independence of children.
- Working closely with multidisciplinary teams to ensure a holistic approach to care.
- Providing guidance and support to families, helping them navigate available resources and services.
