Social Worker - SAS - Hartlepool
We are seeking a dedicated and compassionate Social Worker to join our team in Hartlepool within the Social and Adult Services (SAS) department. This role is essential in providing support and intervention to individuals and families facing various social challenges. As a qualified social worker, you will work collaboratively with clients, families, and other professionals to promote well-being and safeguard vulnerable individuals.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and strengths of clients.
- Developing and implementing tailored care plans that address the unique circumstances of each individual.
- Providing ongoing support and guidance to clients, ensuring their voices are heard and respected.
- Collaborating with multidisciplinary teams and external agencies to coordinate services and resources.
- Maintaining accurate and timely documentation of casework activities and progress.
To be considered for this position, you must possess the following qualifications and skills:
- A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience in adult social care or a related field.
- Strong communication and interpersonal skills, with the ability to build rapport with clients.
- Excellent problem-solving skills and the capacity to work independently as well as part of a team.
