Sefton Team - Social Worker - Locality Team

- Conducting comprehensive assessments of individuals and families to identify their needs and strengths.
- Developing, implementing, and reviewing care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to clients, ensuring they have access to necessary resources.
- Working closely with multidisciplinary teams, external agencies, and community resources to deliver holistic care.
- Maintaining accurate and up-to-date case records in compliance with local policies and statutory requirements.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work or equivalent).
- Registration with Social Work England.
- Proven experience in social work practice, preferably within a locality or community setting.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
