We are currently seeking a dedicated and experienced Qualified Social Worker to join our Assessment team in Hertfordshire (Hemel Hempstead) focusing on children and young people aged 0-25 with complex needs. This is a hybrid position offering the opportunity to work both remotely and in the office, providing flexibility and work-life balance.

Key responsibilities and duties:

  • Conducting thorough assessments of children and young people with disabilities, ensuring their needs are accurately identified and met.
  • Developing and implementing care plans in collaboration with the service users, their families, and multi-disciplinary teams to achieve positive outcomes.
  • Participating in case conferences, reviews, and court proceedings as required, advocating for the best interests of the children and young people.
  • Maintaining accurate and up-to-date records of all interactions and interventions in accordance with regulatory standards.

Essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration.
  • Demonstrated experience in working with children and young people with disabilities, ideally within an Assessment team setting.
  • Strong knowledge of relevant legislation, policies, and procedures related to safeguarding and child protection.
  • Excellent communication, assessment, and decision-making skills to effectively engage with service users and stakeholders.
