Assessment Social Worker - Stockton on tees
We are currently seeking a dedicated and experienced Assessment Social Worker to join our team in Stockton on Tees. As an Assessment Social Worker, you will play a crucial role in assessing the needs of vulnerable individuals and families to ensure they receive the appropriate support and interventions.
Key responsibilities and duties:
- Conducting comprehensive assessments of service users' needs and risks
- Developing and implementing care plans in collaboration with service users and their support networks
- Providing support and guidance to service users to improve their quality of life and well-being

Essential qualifications and skills:
