Social Worker - Care Leavers team - Newcastle
We are seeking a dedicated and compassionate Social Worker to join our Care Leavers team in Newcastle. This role is pivotal in supporting young individuals transitioning from care to independent living. As a Social Worker, you will work collaboratively with care leavers to empower them, ensuring they have the necessary resources and support to thrive in their new environments.
Your key responsibilities will include:
- Conducting assessments to identify the needs and strengths of care leavers.
- Developing and implementing tailored support plans that promote independence and well-being.
- Providing ongoing support and guidance, including assistance with education, employment, and housing.
- Building strong relationships with care leavers, fostering trust and open communication.
- Collaborating with other agencies and stakeholders to ensure a holistic approach to care.
- Maintaining accurate and up-to-date records in compliance with legal and organizational standards.
To be considered for this role, you must possess a recognized Social Work qualification (e.g., degree in Social Work) and be registered with Social Work England. Essential skills include:
